| INTRODUCTION |
The OAA nutrition program serves more clients than any other Title
III program (for more information please go to AoA's
website). Home delivered meals (HDM) clients are often frail and
the survey instrument will measure the extent to which the service
help them remain in the community.

| TEST INSTRUMENT |

The POMP 5 efforts were directed toward revising the nutrition survey
instruments. The revised instruments measure the quality of the service,
the extent to which receiving meals helps maintain or improve clients'
physical well being, and the impact of the service in terms of helping
clients remain in the community.
Because most home delivered meals clients are often frail and receive
other home and community-based services (e.g., personal care) that
help them remain in the community, it is important to collect additional
data on their physical functioning and social and emotional well-being
as well as demographic information to help capture program impact.
