| INTRODUCTION |
Case coordination and service integration have long been an objective
of OAA-funded services, and case management is perhaps the most direct
example of service integration at the local level. Case management helps
to link vulnerable and isolated elders with a range of fragmented formal
services, and can serve as the mechanism for the equitable distribution
of scarce resources.

| TEST INSTRUMENT |
Part of the POMP V effort was the development of a case management
performance measurement tools. The questionnaire for case management clients focuses on
consumer assessment of case management services and the extent to
which the services assist clients in remaining independent. Two versions
of the client questionnaire were developed: one for clients (or translators)
and one for proxies (to answer for cognitively impaired clients).
Additionally, a program administrator's questionnaire was developed with
the purpose of collecting contextual information about the programs,
which the sample of clients are drawn from.
